• Talismans: Crimson Amber Medallion +1, Rotten Winged Sword Insignia, Dragncrest Greatshield Talisman, Erdtree’s Favor +2
• Incantations: Rotten Breath, Ekzykes’s Decay, Scarlet Aeonia
• Wondrous Physick Mixture: Greenburst Crystal Tear and Hidden Cerulean Tear

This is probably the best build that utilizes the Rotten Breath to its fullest potential and lets you play around with other weapons that also cause rot. Weapon

Sacred Seal

Dragon Communion Seal is the best seal that you can use to cast dragon incantations, including the Rotten Breath. It basically boosts all types of dragon incantations, and it additionally scales off Arcane. This is extremely useful as both weapons and the seal scale off the same stat. Both of these will be used to inflict scarlet rot on your enemies additionally.

Talismans

Prioritize Vigor and Arcane for the rot build. I suggest you to use the Crimson Amber Medallion +1 to boost maximum health, ensuring prolonged survival as scarlet rot affects enemies. The Rotten Winged Sword Insignia increases the damage output of the weapon with successive attacks. These include Scarlet Aeonia and Ekzykes’s Decay. You should note that you’ll have to invest more in the Faith stat in order to use some of these higher-level rot incantations. But you won’t need to use them, as Rotten Breath is already very strong in the game.

Wondrous Physick Mixture

The Hidden Cerulean Tear will negate all FP consumption for a limited amount of time. You can use that to launch a Rotten Breath as soon as you enter a boss battle to get the upper hand by inflicting scarlet rot on the boss. And the best part is that no FP will be consumed for that first Rotten Breath.

The Greenburst Crystal Tear boosts your stamina recovery speed. You’ll need that as the Rotten Breath heavily drains your stamina after a single use, so it will come in handy to have better stamina recovery speed. It just makes your fights safer and comes in clutch when you least expect it.
